Nursing Care

Community Nursing Care is an essential service that YH Support Services provides to people with disabilities. This service is designed to support Participants in managing their health and well-being, and to ensure that they have access to the care they need in the community.

Our Community Nursing Care services include:

• Providing nursing care: We provide nursing care to Participants, including administering medication, monitoring vital signs, and providing wound care. Our nursing staff are highly trained and experienced, and are committed to providing high-quality care in a person-centred way.
• Supporting with health management: We provide support with managing Participants’ health, including helping Participants to understand their conditions, monitoring symptoms, and making any necessary adjustments to their care plans.
• Assisting with accessing healthcare services: We assist Participants in accessing healthcare services, including scheduling appointments, accompanying Participants to appointments, and providing transportation.
• Providing education and training: We provide education and training to Participants, their families, and caregivers, to ensure that they have the knowledge and skills they need to manage their health and well-being.
• Providing ongoing support and review: We provide ongoing support and review of Participants’ progress, and make any necessary adjustments to their care plans as needed.

At YH Support Services, we understand that managing health and well-being can be a significant challenge for people with disabilities. Our services are tailored to each Participants needs, and are provided in a person-centred way, working closely with Participants, their support networks, and other stakeholders to ensure that our services meet their individual needs.
