Assist-Life Stage, Transition
Assist-Life Stage, Transition is one of the many services that YH Support Services offers to people with disabilities. This service is designed to support Participants through significant life changes and transitions, such as moving out of home, starting school or college, or moving into a new job or home.
At YH Support Services, we understand that life transitions can be difficult and challenging, especially for people with disabilities. That’s why we are committed to providing high-quality Assist-Life Stage, Transition services that support Participants through these changes and help them to achieve their goals and aspirations. Our services are tailored to each Participants needs, and are provided in a person-centred way, working closely with Participants, their support networks, and other stakeholders to ensure that our services meet their individual needs.
Our Assist-Life Stage, Transition services include:
- Assisting Participants with planning and preparation: We work with Participants to identify their goals and aspirations for the future, and then assist them to plan and prepare for the transition. This may include helping them to identify the skills and resources they need, and providing support to acquire them.
- Supporting Participants during the transition: We provide practical and emotional support to Participants during the transition, helping them to navigate the changes and challenges that they may face. This may include providing support with housing, employment, education, or other areas of their life.
- Assisting Participants to connect with relevant services and supports: We assist Participants to connect with relevant services and supports that can help them during the transition. This may include connecting them with mainstream services such as schools, colleges, or employers, or connecting them with disability-specific services such as disability employment services or disability accommodation services.
- Assisting Participants to maintain their independence: We provide support to Participants to help them maintain their independence and autonomy during the transition. This may include providing support with personal care, domestic tasks, or other areas of their life that they may need assistance with.
- Providing ongoing support and review: We provide ongoing support and review of Participants’ progress during the transition, and make any necessary adjustments to their support plan as needed.
