We are your trusted and compassionate Registered NDIS Services Provider, dedicated to enhancing the lives of individuals with disabilities. With our roots in Adelaide SA, and a thriving branch in Sydney NSW. Our management team has over 15 years of experience in working with people with disabilities and has a wide range of experience working with individuals with physical disabilities, Down Syndrome, Autism, Alcohol Foetal Syndrome, Bi-Polar disorder, Schizophrenia, Dementia, depression, and people experiencing mental health issues.

Our team at YH Support Services works closely with our Participants and their support network, including OPA, PT, PBS Practitioners, Plan Nominees, Physio Therapists, Occupational Therapists, Mainstream supports, and psychologists. We actively participate in case conferences and keep all stakeholders up to date with our Participants’ progress. Our Support Coordination team will assist our Participants to connect with Allied Health Professionals (AHP) that will help them to live as independently as possible. Our Direct Support Workers will follow the support plans prescribed by the AHP and work with our Participants to achieve their goals. The support plans will be reviewed every 6 months or as required to track progress and make necessary changes.

In addition, we encourage our Participants to be involved in the decision-making process and to have a say in the support they receive. We believe that involving our Participants in the decision-making process empowers them and enables them to take control of their own lives.

YH Support Services provides extensive and detailed documentation related to our Participants’ goals, which complies with the NDIS plan review process. We have a robust business system to store our Participants’ information and it can be retrieved safely and securely when required. Privacy and confidentiality are of the utmost importance to us at YH Support Services. All of our staff are compliant with the NDIS requirements and have gone through the checking and verification process.

We offer a wide range of services, including Accommodation/Tenancy, Assist-Life Stage, Transition, Assist-Personal Activities, Assist-Travel/Transport, Community Nursing Care, Daily Tasks/Shared Living, Innovative Community Participation, Development-Life Skills, Household Tasks, and Participation in Community Group/Centre Activities.

Our recruitment process begins with an initial screening of applicants, where we assess their qualifications, experience, and suitability for the role. We only consider applicants who have relevant qualifications and experience in working with people with disabilities.

After the initial screening, we conduct a thorough background check on all shortlisted applicants. This includes a check of their criminal record, work history, and any relevant references. We also conduct NDIS worker screening checks to ensure that all our staff are compliant with the NDIS requirements.

Once we have completed the background check, we invite the shortlisted applicants for an interview. During the interview, we assess their skills, experience, and suitability for the role. We also take into account their attitude, communication skills, and their ability to work as part of a team.

After the interview, we conduct a final selection process, where we consider all the information we have gathered during the recruitment process. Only applicants who meet our strict criteria are offered a position with YH Support Services.
